City of York Council have an existing opportunity for a social worker to join the Hospital Social Work Rehabilitation Team.
The Hospital Social Work Rehabilitation Team is a committed, friendly and forward -thinking team, working alongside Discharge Liaison to support customers with discharges from non-acute rehabilitation units White Cross and St Helens in York.
Our aim is to put social work at the heart of everything we do and to take a strength based approach and ensure customers receive the personalised support they need to facilitate safe and timely discharges from non-acute hospitals.
We work very closely with our health colleagues within White Cross and St Helens and partner agencies to support customers to live as independently as possible. You will be responsible for assessing customers' needs in line with the Care Act, assessing capacity, undertaking Safeguarding enquiries. You will be part of the Complex discharge Hub where you will discuss customers delaying and support the manager to report on Delayed Transfers of Care (DToC). You will arrange /attend discharge planning meetings, Best Interest meetings and complete Continuing Health Care Assessments within the customers own home or in a care home as part of the Discharge to assess pathway. You will undertake reviews following discharges from hospital.
Working within a fast paced multi- disciplinary team you will need to have excellent communication skills, be organised, provide professional advice and support, be able to build positive relationships, have the ability to work confidentially under pressure and case hold. You will need to have excellent IT skills.
The successful candidate will hold a relevant professional qualification in Social Work, have relevant experience and show enthusiasm about working with Adults. You will need to be registered with the HCPC.

As this role is public facing, applicants will need to demonstrate, at interview, their competency to converse and provide advice and guidance to members of the public, in spoken English to CEFR level C2: Can express him/herself spontaneously at length with a natural conversational flow, avoiding or backtracking around any difficulty so smoothly that the person with whom they are conversing is hardly aware of it.

The postholder will have access to secure government sites or email. In order to use these systems, the Cabinet Office requires the HR Team to carry out Baseline Personnel Standard Security (BPSS) checks.
