City of York Council Hospital Social Work Team has an exciting opportunity for a social worker to join the team as a renal social worker.
You will be joining an enthusiastic multi-disciplinary team working with people with kidney disease and their families. Based within York Hospital you will be working alongside another renal social worker, a team of doctors, nurses, dieticians, pharmacist and psychologist.
You will manage a caseload of customers who are under the care of the renal unit. You will be responsible for assessing individual needs in line with the Care Act, assessing capacity and making best interest decisions when required, undertaking safeguarding enquires, continuing health care assessments, providing information and advice, and supporting with benefits advice and appeals.
You will work to enable people to maximize their potential and live with dignity and independence in their own homes and communities. This should be achieved using professional social work skills, providing appropriate services and supporting carers and community networks, within available resources. You will commission and review care packages both in the hospital and community setting.
As a renal social worker you will work with patients; before they come to dialysis, those on dialysis or who are being managed conservatively, patients who have had a transplant and those at end of life. You will give advice and support on a range of emotional and practical matters, working with patients at home, in the hospital or on the renal unit.
The post is funded by York Foundation Hospital Trust but you will be supervised by a manager in the Hospital Social Work team and employed by the City of York Council.
The successful candidate must hold a Diploma in Social Work (or equivalent professional social work qualification) and have a minimum of 2 years post qualifying experience. Be registered with the HCPC and have a driving license is essential.
As this role is public facing, applicants will need to demonstrate, at interview, their competency to converse and provide advice and guidance to members of the public, in spoken English to CEFR level C2: Can express him/herself spontaneously at length with a natural conversational flow, avoiding or backtracking around any difficulty so smoothly that the person with whom they are conversing is hardly aware of it.
