To support our work around the Transforming Care Programme, we are seeking to appoint two Specialist Designated Practitioners. You will be committed to providing real time support and intervention to those on the Learning Disability Dynamic Support Register to avoid hospital admission and ensure timely discharge.
Do you will have a minimum of 2 years post qualifying experience and will have worked with people with a Learning Disability and Autism? As part of the role you will work closely with our commissioning partners and key stakeholders in health and Social Care.
You will be adept at managing high demands, with strong organisational abilities and initiative. You’ll manage pressure and competing tasks well, while being able to identify opportunities for innovation to improve what we do and how we do it.
With an excellent understanding and ability to apply legislation including the Mental Capacity Act, Care Act, Mental Health Act and Equality act, and frameworks such as the NHS Continuing Health Care Framework you will be highly skilled at providing support via signposting, undertaking Assessments, Support Planning and Reviews. You will have strong reasoning and sound decision making skills, be able to receive and provide professional challenge, and make recommendations for a course of action for the Local Authority that is legally and ethically defensible. You will also know when to seek further advice and guidance and be proficient at using new information to reformulate professional opinions.
It will be important to you to have or to develop a range of methods to work with people who may have different ways of communicating to ensure capacity and participation in decision making is maximised.
You will work collaboratively with peers, including those in the leadership team to support the Local Authority to meet its statutory requirements and vision to support people with care and support needs to have a good life, free from abuse and neglect.

At the moment due to the impact of the ongoing COVID-19 pandemic, some of our staff are working remotely from home but a return to a combination of working remotely and physically from the Council’s offices is anticipated in the near future.
As a new starter, your contractual location will be the designated council office for your team. Your working arrangements will vary according to your service and role, and the current Government guidance. For many roles, there will be more opportunities in the future for remote and hybrid working. However, this comes with the expectation that, as workplaces open up, there is likely to be an ongoing and occasional requirement to attend the workplace.
As this role is public facing, applicants will need to demonstrate, at interview, their competency to converse and provide advice and guidance to members of the public, in spoken English to CEFR level C2: Can express him/herself spontaneously at length with a natural conversational flow, avoiding or backtracking around any difficulty so smoothly that the person with whom they are conversing is hardly aware of it.
